Wow! It appears that you can actually get three different $5/$25 coupons if you take the flu survey from the following three different links:
Each of these have unique bar codes, (be sure to check, as I have not tried) so you should be able to use each of them once per card!
Tried two of the three links. I've already printed off the middle link. The top and bottom link yielded coupons with different bar code numbers.
